Readiness For Treatment For Perfectionism
Participant Information Sheet
Participants must be 18 years or over to participate in this research
My name is Dolores Elek-Roser. I am currently completing the Doctorate of Philosophy (Psychology) degree at Curtin University of Technology, Western Australia. My supervisors are Dr Sarah Egan and Dr Lynne Roberts and my associate supervisor is Prof Jan Piek. As part of my research I am developing a measure to assess readiness for treatment for perfectionism. Your participation in this research will help me develop and assess the new measure for readiness for treatment for perfectionism. While this study will not benefit you directly it will add to a growing body of research and further understanding in this area.
Your participation is entirely voluntary, and you may withdraw at any time. You will be asked to complete all questions on a questionnaire. The questions relate to your mood, factors that influence setting personal standards, your attitude about seeking treatment, and demographic information. To ensure anonymity no personal identifiable information will be included on the questionnaire. In addition, confidentiality with all the data collected will be assured at all times. Data collected will be averaged for analyses and may be used in future academic publications and presentations. By clicking on the ‘consent’ button you are consenting to participate in this study.
It is important that all questions are answered. The questionnaire will take approximately 15 minutes to complete. Once you have submitted the questionnaire you have the chance to enter a draw for a $50.00 on-line gift card from Amazon. com. Your email details will be housed on a separate server to the questionnaire and cannot be matched.
If you have any concerns about this study please feel free to contact me. I can be contacted at email@example.com or by telephone, on (international dialling code 0011618) 9266 3446. Dr Egan can be contacted at S.Egan@exchange.curtin.edu.au or by telephone (international dialling code 0011618) 9266 2367.
It is not expected that completing the questionnaire will cause any distress. However, if you experience any and would like to talk to someone about seeking psychological treatment contact your local psychological or counselling service provider.
Thank you, Dolores Elek-Roser
